You can take a picture of a woman sticking out her tongue, break the image apart, blend the middle of her tongue to match the background (In this case, her lips) and then import the zipper image, blend and touch the image up and there you go.

But, there might be a chance it's real too. With most Photoshopped pics, there's sometimes a slight error in the pic.
