Minor correction: Your initial clearance is granted through a background check. Your clearance is renewed or updated periodically, and that will require a limited back ground check.
Higher level clearances are more dependent on your position and your "need to know", they don't necessarily do a full background check for each level.
There never was a person who was "QAnon".
The "QAnon" conspiracy thing was created by three guys. Two were administrators on 4Chan, the other was just one of their friends.
There never was a person in the government who was the source of the "QAnon" information... these three guys were the source... initially.

To keep a clearance is dictated by what you do while you have it. For instance if you pick a bar fight with SEALS they will probably walk away. If they get busted, and convicted, for assault they risk losing their clearance. Their clearance is suspended in the interim before trial. To get a higher clearance the first investigation is used. There are some factors that can limit a persons clearance level. In those cases the highest clearance they can get will determine what jobs they cannot be assigned.
QAnnon started out as a person, was hijacked by a person or persons bent on destroying and delegitimizing the concept of a "deep state." It is also interesting, and purposefully misleading. the supposed operatives of the "deep state" were painted out to be liberals.
