I know a guy who had an accident with a bench saw at work when he accidentally cut off two of his fingers. When he came back to work his boss asked him to show him how the accident happened so that they could ensure a similar accident didn't happen in the future.
The plonker lifted a piece of wood and pushed it through the saw with his bare hands instead pushing it with another piece of wood. Yes you guessed it. He cut off two more fingers.
